COMPULSORY TASK:
To create a presentation or document which will make up the basis of the first part of your coursework for year 13:
For your Unit 3 coursework, you will refer all your tasks to two individual case studies.
Individual 1 – Shahfat
Shahfat is 72 years old and is in the hospital following a stroke. She is a widow and lives with her daughter and son-in-law. They are all practising Muslims. Before the stroke, Shahfat was independent and used to care for her five grandchildren. Following the stroke, Shahfat is having problems speaking and, due to weakness on one side of her body, is unable to mobilise. While in hospital, Shahfat is visited by her daughter every day, although the family have said they will not be able to care for her at home.
Individual 2 – John
John is 24 years old and lives with his parents. He has spent some time under the care of the community mental health crisis team, but has recently stopped responding to their phone calls. When they call at his parents’ house, they find out that he has not been at home for a few days, and his parents are concerned that he has started drinking heavily/taking drugs again. When talking to his parents, the team find out that they are also struggling to cope with helping to care for John.

- Everyone counts
Apply NHS Core Values to Case Studies:
- 2 NHS Core Value for Shahfat
- 2 NHS Core Value for John
For Merit (M1):
- Explain why the NHS Core Values are important
- How do the NHS core values build trust and relationships
- Discuss how effective the NHS Core Values are in providing person-centred care
- Discuss the effectiveness or limitations of the NHS Core values
Do this in general and link it to the case study individuals.
Skills for Care Values (P1 and M1) - Explain each of the Skills for Care Values:
- Dignity and respect
- Learning and reflection
- Working together
- Commitment to quality care and support
- Apply Skills for Care Values to Case Studies:
- 2 Skills for Care Value for Shahfat
- 2 Skills for Care Value for John
For Merit (M1):
- Explain why the Skills for Care Values are important
- How do the Skills for Care Values build trust and relationships
- Discuss how effective the Skills for Care Values are in providing person-centred care
- Discuss the effectiveness or limitations of the Skills for Care Values
Do this in general and link it to the case study individuals.
The 6Cs (P1 and M1) - Explain each of the 6Cs with definitions and examples:
- Care
- Compassion
- Competence
- Communication
- Courage
- Commitment
Apply the 6Cs to Case Studies:

For Merit (M1):
- Explain why the 6Cs are important
- How do the 6Cs build trust and relationships

OPTIONAL EXTENSION:
Overcoming Challenges in Providing Person-Centred Care (D1)
For D1: You must draw everything together; this means combining everything discussed in Activity 1 Values. You should link the values together.
The main focus in how professionals successfully provide person-centred care.
- You need to think about:
- Which values work best?
- Why are they effective?
- What challenges exist?
- Which values improve outcomes the most and why?
You should:
- Make judgements
- Compare values
- Explain which values are most effective and explain why
- Discuss challenges and limitations
- Link back to outcomes (trust, wellbeing, recovery, independence, safety)
- Apply this constantly to BOTH case studies
